The Humber Students’ Federation is a proud sponsor of the HSF Health and Dental Plan. The following information will aid you in learning more about the HSF Health plan and the many services it offers.



The HSF is proud to provide students with an extended health and dental plan. Please see below for highlights of the improvements to your plan.

  • Every full-time fee-paying registered student at Humber Institute of Technology and Advanced Learning is automatically covered for the benefits of the Health and Dental Plan
  • Coverage is only for the semesters that you are registered full-time and have paid all of your fees. Coverage terminates at the end of the policy year. The policy year runs from September-August.
  • If you drop out or leave for any reason, the coverage is automatically terminated.
  • The Health Plan is mainly used for prescriptions; however it covers a number of other things.

Please Note: For student only attending for one semester, be aware that your coverage is only for that semester. September semester runs September 1, 2007 to December 31, 2007, January semester runs January 1, 2008 to April 30, 2008 and Summer semester runs May 1, 2008 to August 31, 2008.

 

The HSF offers a dental plan to all full-time students. Students should review the coverage booklet for benefits coverage entitlements PRIOR to opting out of this plan. Once a student has opted out of the dental plan, they cannot re-enroll.

If you are a full-time student wishing to opt-out of the dental plan, you may now opt-out online.

How to Make a drug/dental claim?

  • Present your Humber student identification card to the pharmacist or dentist across Canada and payment of eligible claims will be covered. To fill a prescriptions drug or dental claim, you will need to supply the pharmacist/dentist with the following information:
    • Your Group Number is 514561
    • Provider: ClaimSecure (formerly RXPlus/Merz Health Corporation)
    • Your Student ID#Z________(10 digit alpha numeric number)

I.E. if your student ID # is 8 digits, the correct ID# would be z087654321
I.E. if your student ID # is 9 digits, the correct ID# would be z987654321

At this point you will be required to pay the deductible amount of your claim if necessary. Please note the dental office may charge more than the Fee Guide, which will require you to be responsible for any additional costs.

Students will be required to pay 20% per prescription as the Health plan only covers 80% of prescription drugs.

For Other Claims:

  • All other claims must be submitted on the Claim Submission Form Available online or at the HSF offices.
  • The form must be submitted within 30 days of the date of the accident.
  • Receipts and statements from doctors must be submitted upon request.


Special note to Placement and Internship Students

Placement students are not covered under the Health and Dental Plan.

This is because the full-time activity fee is not paid to the HSF. The fees you do pay relate to internship/placement administration costs. If you are interested in opting in to the insurance plan during your placement/internship semester, the cost of the plan is $101. Please email info@hsfweb.com for information.


Out of Country Provision if Traveling Abroad


Students traveling abroad should inquire about out of country entitlements prior to traveling abroad. The student may want to consider buying travel insurance for added protection.
